During your pre-surgery health assessment, our staff will:
- Take your medical history.
- Perform a physical exam and possibly order bloodwork or an EKG to ensure surgery safety.
- Review any tests ordered by your doctor.
- Discuss the medications you should or should not take before surgery.
- Order any additional test(s) that can help make sure it’s safe to proceed with surgery.
What to have ready for your pre-surgery assessment
- Any patient intake forms we’ve asked you to complete.
- Your insurance card and co-payment, if applicable.
- Contact information for your primary care provider and any other specialists you see regularly.
- Medical records related to any previous major medical evaluations.
- An up-to-date list of your medication(s) and supplement(s) in their original packaging if possible. Please share with us the drug name and dosage, number of pills you take at once, and how often you take them. Please also inform us about any over-the-counter medication, herbal or other supplements you take, including dosage and how often you take them.
- Patients must sign a surgical consent form before any medication can be given or surgery performed.
- Minors must have a parent or legal guardian sign a consent form before surgery can be performed.
- Patients should not leave the Center unless they are under the care and supervision of a responsible adult. This may be a friend or relative who can drive the patient home.
- Smoking 24 hours before or after surgery may be detrimental to your recovery.
- Arrange to have a responsible adult drive you home after surgery. You will not be permitted to drive home after surgery or drive for 24 hours. Do not plan to use taxis, rideshare, or public transport as they are not responsible for getting you home safely.
- Once discharged, plan to have a relative, friend, or dedicated caregiver stay with you the day of your surgery and overnight to oversee your recovery.
If needed…
- Children and dependent adults: A legal guardian must be inside the surgery center at all times for children and dependent adults.
- Childcare after surgery: If you have young children at home for whom you normally care for, plan for someone else to care for them for at least the first 24 hours following your surgery.
- Interpreter: If the patient cannot read or speak English, please arrange to have a translator who can explain pre-surgery instructions and ask/interpret any questions the patient may have.
Please follow the instructions below regarding your medication prior to surgery. If you have questions regarding your medication, please contact your doctor’s office.
Review and comply with any medication instructions given to you during your pre-surgery appointment. If you are taking a medication or supplement that we have not recorded during health intake, please call us and inform us as soon as possible.
Please follow your surgeon’s instructions regarding which medications, vitamins, supplements to hold prior to your surgery.
- The following medications and supplements must not be taken for 14 days prior to surgery: Fastin, Ginko Biloba, Ginseng, Ionamin, Meridia, Metabolife, Phenteramine, Pondimin, Redux, Saint John’s Wort, Valerian, Kava, or Mahuang.
- If you are taking a blood thinner (such as aspirin, Coumadin, or vitamin E), make sure your surgeon is aware. Follow your doctor’s instructions regarding the dose.
- If you are taking aspirin, Motrin, or an anti-inflammatory medication, make sure your surgeon is aware. Follow your doctor’s instructions.
- If you have asthma or lung disease, use your inhalers and/or breathing treatment the morning of surgery and bring the inhalers with you.
Ambulatory Surgery Center of Utah is located at 5782 Adams Avenue Parkway. The Surgery Center is on the lower level of the building, park in the lower parking lot.
